Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire.  This should be done at least 6 weeks before you are due to travel. The form includes details of which countries and areas within countries that you are visiting.  The Practice Nurse will then check your records to see which, if any vaccinations you require  There is further information about countries and vaccinations required on the links below

It is important that you return this form as early as possible  - at least 6 weeks before you travel.  Where your travel date is less than 6 weeks ahead you will be given details of local Private Travel Clinics.  Travel vaccines have to be ordered as they are not a stock vaccine.  Your vaccination appointment should be at least 2 weeks before you travel to allow the vaccines to work.

Please take into consideration peak times and staff holidays as it is only the Practice Nurse that can carry out these travel checks.

Some travel vaccines are ordered on a private prescription and these incur a charge at the Pharmacy.  This is because not all travel vaccinations are included in the services provided by the NHS.
